Pursuant to Article 13(1)(2) and Article 14(1) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the use of personal data and on the free access to such data and the use of services 95/46/EC (hereinafter referred to as the "GDPR"), we hereby declare that

I.

The Controller of your personal data is Aleksandra Smoczyńska, operating under the name OLA Interpreting Aleksandra Smoczyńska, with its registered office in Warsaw, Kobiałka 40, 03-044 Warsaw.

II.

You can contact the Controller by post at the following address: Kobiałka 40, 03-044 Warsaw, or by email at the following address: [.].

III.

We will process your personal data for the purpose of providing "On Demand Interpretation" services.

IV.

Data not obtained directly from you includes the following categories of data: name and surname, contact details, place of residence or stay, and job title.

V.

The source of personal data not directly related to you is the entity with which the Controller connects access to the "On Demand Interpretation" services.

VI.

The risk of using your personal data is:

  1. Article 6, paragraph 1, letter b of the GDPR, i.e., for the purpose of fulfilling the "On Demand Interpretation" service agreement.

VII.

The Controller has carefully verified your personal data:

  1. name and surname,

  2. contact details, such as email address, telephone number, and home address,

  3. place of residence or stay,

  4. job title.

VIII.

The recipients of your personal data are entities with which the Controller has a contract for the provision of services: accounting, advisory and consulting, legal, and regulatory.

IX.

Your personal data will remain for the duration of the "On Demand Interpretation" service, pending its delivery and settlement, and under separate law, the retention period for accounting and bookkeeping documentation, which is no less than 5 years from the end of the year in which the VAT invoice for the on-demand interpretation was issued.

X.

You have the right to:

  1. request access to your personal data from the Controller and obtain a copy thereof,

  2. request rectification of your personal data,

  3. restrict the processing of your personal data,

in accordance with the basic principles set out in Articles 15, 16, and 18 of the GDPR.

XI.

Furthermore, if your personal data is processed based on consent, you have the right to:

  1. erase your personal data,

  2. transfer your personal data,

  3. object to the processing of your personal data,

in accordance with the basic principles set out in Articles 6, 17, 20, and 21 of the GDPR.

To exercise the aforementioned rights, please contact the Controller at the Controller's address in Section II above.

XII.

You have the right to file a complaint with the President of the Personal Data Protection Office if you believe that your personal data is being transferred in violation of the GDPR. Address: President of the Personal Data Protection Office, ul. Stanisława Moniuszki 2A, 00-014 Warsaw.

XIII.

With respect to your personal data, which is not subject to automated decision-making.

XIV.

Providing personal data is required, but is supplementary to the "On Demand Interpretation" service certification agreement and is necessary for the implementation and proper performance of the aforementioned agreement.
